The David and Lura Lovell Foundation and Community Foundation for Southern Arizona have jointly awarded $3.6 million to 12 nonprofits in the Arizona End of Life Care Partnership that are cooperatively addressing end-of-life care issues in Tucson, Arizona.

“Meet and Complete”

Are you over 18? Have you completed your advance directives?

• Living Will—Your decisions about the types of healthcare treat-ments you want or do not want as you approach the end of life

• Durable Power of Attorney for Healthcare—Your choice of a spokesperson to make healthcare decisions for you if you become incapacitated

• “Do Not Resuscitate”—Optional: Your declaration that you do not wish to be resuscitated if your heart or breathing stops

If you either have not finished your advance directives or would like to revise the ones you have, we’re available to help you with these.**
