Durable Power of Attorney for Healthcare—Your choice of a spokesperson to make healthcare decisions for you if you become incapacitated.
Living Will—Your decisions about the types of healthcare treatments you want or do not want as you approach the end of life.
“Do Not Resuscitate”—Optional: Your declaration that you do not wish to be resuscitated if your heart or breathing stops.
