Advance Care Planning “Meet & Complete”

Advance care planning is a way to help insure that your values in life and wishes for care will be honored all the way through your life, if you are ever unable to speak for yourself.  Have you had your advance care planning conversation but need help with the next step?  Want to get started on your advance care plans but not sure where to start?  We are here to help.

Attend a ‘Meet & Complete’ and get help with taking the next step…

Durable Power of Attorney for Healthcare—Your choice of a spokesperson to make healthcare decisions for you if you become incapacitated.

Living Will—Your decisions about the types of healthcare treatments you want or do not want as you approach the end of life.

“Do Not Resuscitate”—Optional: Your declaration that you do not wish to be resuscitated if your heart or breathing stops.
