AzHHA: Communication in Serious Illness Train the Trainer

Communication in Serious Illness - Train the Trainer

Target audience: healthcare teams such as physicians, nurses, social workers and other healthcare providers.

This 5.5-hour workshop trains physicians, nurses and social workers to facilitate the 2.5-hour Communication in Serious Illness workshop. This session will review a system to identify appropriate patients, training and coaching resources for clinicians, a prompt for clinicians to conduct serious illness conversations at the right time, how to use a short, simple Serious Illness Conversation Guide, suggestions to help patients discuss preferences about their care with families, and a system for documenting personalized care goals and  priorities. This session includes skills practice/role play with feedback and debriefing, Train the trainer expectations will be to successfully co-facilitate training with an approved trainer, submit all required paperwork and commit to co-host future training sessions using set curricula.
