The David and Lura Lovell Foundation and Community Foundation for Southern Arizona have jointly awarded $3.6 million to 12 nonprofits in the Arizona End of Life Care Partnership that are cooperatively addressing end-of-life care issues in Tucson, Arizona.

Honoring A Life: Advance Care Planning 
Train the Trainer

Thoughtful Life Conversations and Arizona Hospital and Healthcare Association are pleased to offer this 6-hour workshop to train community representatives and volunteers to facilitate the 2-hour Honoring A Life: Advance Care Planning Conversations workshop. The participants will observe a demonstration of the presentation, learn facilitator techniques and strategies, review supplemental facilitator notes and slides, participate in engagement activities, and review Arizona documents and community/state/national resources in order to successfully lead the 2-hour advance care planning workshop. Train the Trainer
expectations will be to successfully pass a post test, successfully co-facilitate training with an approved trainer, submit all required paperwork and commit to co-host future training sessions using set curricula.

Target Audience: Volunteers who have a passion for teaching advance care planning workshops.
